Patient Information and Forms

Whether you are coming to Central Florida Dermatology for evaluation of a skin disorder, for skin cancer surgery or for a cosmetic procedure, we want your visit to go as smooth as possible.  Please review the information below about what to bring with you to your appointment, what insurance plans are accepted at our office, and the office policy on financial payments and referrals.  Be sure to arrive at least fifteen minutes before your appointment time if you are a new patient or you have any changes to your insurance or personal information.

What to Bring

  • List of your medicines
  • Phone number of your preferred pharmacy

Insurance Plans

We are contracted with most major insurance plans, including:

  • Healthchoice PPO
  • Healthchoice Select
  • Multiplan
  • Medicare
  • Blue Cross Blue Shield PPO/Blue Options/Traditional
  • Aetna
  • BeechStreet PPO
  • Private Healthcare Systems (PHCS) PPO
  • First Health Mailhandlers
  • Coventry Health Plans

The above list is not all-inclusive. Please contact your insurance company to verify that we are a participating provider prior to your visit. We will file claims to PPO, Commercial and Medicare supplementary insurance plans with which we are contracted providers. Depending on the coverage of your insurance plan, you may be eligible for out of network coverage even if we are not providers for your insurance.


As required by your insurance plan, payments are collected at the time of your visit.  Please arrive prepared to make your payment.  If you are paying out-of-pocket for your health care or cosmetic service, payment will be collected on the date of service.  For your convenience, we accept cash, check, or credit card (Visa and MasterCard).



For those patients whose insurance company requires a referral to our office, you must bring a copy of the referral on the day of the appointment.  Patients whose insurance company requires a referral that arrive without a valid referral on the day of their visit will be asked to reschedule.  Unfortunately, we will be unable to assist you in obtaining a referral at the time of your visit.

We will not try to sell you personalized skin care products.
